Have your children seen for an annual physical exam and review their immunizations.
A physical exam, signed and stamped by a licensed provider and dated within one year prior to registration, is required for each student initially entering a Florida school. This is proof that body systems (heart, skin, lungs) were examined.
- DOH-Pasco will NOT be providing school physicals for the 2022-23 school year.
Up-to-date immunizations are required, and all immunization dates must be on Florida Certificate of Immunization (DH680). Contact your physician or the DOH-Pasco for the form.
- DOH-Pasco clinics provide free childhood immunizations. Appointments are required.
