All 2024 Football Season paperwork is due June 7th. Athletes will need to use www.athleticclearance.com.
Physicals are on a NEW form this year, you will be required to RENEW before participation. Please check to make sure you have a good physical. Upload new physical on the athletic clearance website.
Renew any parent information, medical information, signatures (Pasco County Code of Conduct, EL3 forms).
Upload the following:
- EL2 Physical Form (Page 4 Only)
- Athletic Participation form signed electronically (new)
- Transportation release (please select all options for transportation)
Complete the following:
- Concussion
- Heat Illness
- Sudden Cardiac Arrest

July: 8-11, 15-18, 22-25, 29-31
August: 1 & 2
