All SLHS students planning to attend the SLHS Prom 2025 must complete the Student Agreement Form. This form is linked here. Please note that ticket purchases through Rycor are non-refundable.
Please make sure you meet the following criteria to attend:
- Seniors or Juniors only (unless going as a guest)
- GPA: You must have a 2.0 or higher
- Absences: No more than (5) unexcused absences in a month or (10) unexcused absences within a semester (in this case, semester 2)- this includes absences per period Florida Law (Section 1003.26(1)(b), Florida Statutes) provides that if a student has at least five (5) unexcused absences, or absences for which the reasons are unknown, within a calendar month or ten (10) unexcused absences, or absences for which the reasons are unknown, within a ninety (90) calendar day period, the school must take action. The school may require documented medical and legal excuses for a pattern of non-attendance. • Attendance shall be counted period by period (Secondary only) • After three (3) unexcused absences the school shall contact the home (or workplace) of the parent/guardian to determine the reason for the absence
- Tardies: No more than 5 unexcused tardies in any given class in semester 2—those with excessive tardies will need to go through the appeal process with their administrator
- Referrals: If you have any Level 2 or level 3 referrals, you will need to complete an appeal form for your grade level administrator
Non-SLHS students are welcome to attend as guests of SLHS students. Each SLHS student is permitted to bring one guest. To bring a guest, the SLHS student must submit a completed Guest Request Form, along with a photo ID or student ID of the guest attached. Forms submitted without photo identification will not be accepted, causing delays in the approval process. This may result in the guest being unable to attend the prom. The Guest Request Form is available for download below, or a hard copy can be obtained in Student Services.
All forms must be submitted by Wednesday, April 30, 2025, at 2:00 pm—late forms will not be accepted.

Appetizers, dessert, and drinks will be provided. Parking is free. Doors will close promptly at 9:00 pm for prom entrance.
